What is collected
When you use a form on this site, you may share your name, email address, written answers, assessment responses, and the report scores produced from them.
Why it is used
Your details are used to deliver the requested report, understand the context behind it, and respond to your submission. Occasional field notes are sent only when you select that option.
Where it goes
Form submissions are processed and stored through this site's hosting provider. Your information is not sold. It is kept only for the report, the conversation that follows, and any updates you asked to receive.
Your choice
You can ask to see, correct, or delete your submission, or stop receiving field notes, by writing to gi@dkstudio.ai.
